Please send us a message from the "contact" page. We offer services within the local PCSD area at no cost to you. Appointments are required and must be confirmed 24 hours in advance.

Any change in office hours will be updated on Google, Facebook and Instagram
Starting on January 1st, 2026, we will no longer accept Caresource Medicaid. Caresource is moving from Superior Vision and will now be serviced by EyeMed Vision Care
New plans added include Premier Eye Care, AFSCME Care Plan, and AmeriHealth Caritas Ohio
We do not accept the following vision plans:
